Suicide ‘signs were missed’
A CORONER found Camden and Islington Mental Health Trust “missed opportunities” to assess a patient who later committed suicide and said it failed to keep to an “expected standard” of practice.
Yaroslav Sydor, 61, a trade union leader from the Ukraine, slashed his arms while sectioned at the Grove Centre in Fleet Road, Hampstead in August.
Mr Sydor left his wife and children behind when he fled his home country over fears he was being pursued by the KGB and MI5 for his union activities.
An internal review carried out by the trust into his care found that staff had failed to stick to guidelines and uncovered evidence of poor record keeping.
St Pancras Coroner Dr Andrew Reid heard at an inquest last Wednesday that assessment of his mental state was missed because of a mix-up between different wards at the centre.
Verdict: Suicide. |
